Brain fog can be alarming.

You may feel like you cannot recall familiar words. You may lose your train of thought in the middle of a conversation or find yourself more easily distracted. During the menopause transition, 40% to 60% of women report cognitive complaints, including difficulty remembering, finding words, concentrating, or staying focused.

For some women, these changes can be unsettling enough to wonder, “Is this menopause, or is something more serious happening?”

Brain fog is not the same as dementia.

Dementia involves a more global decline in cognitive functioning, including thinking, remembering, and reasoning. Depending on the type of dementia, changes can also involve language, judgment, problem-solving, attention, perception, behavior, and personality.

Brain fog during the menopause transition can absolutely affect daily life.

You may notice that your thinking doesn't feel as sharp as it once did. You may have difficulty retrieving a familiar word, concentrating during a conversation, remembering information, or managing several things at once. You may notice changes in executive functions such as planning, organizing, and shifting your attention.

For a woman who is used to performing at a high level, these changes can be particularly frustrating. You may still be highly capable, successful, and functioning independently while noticing that your brain isn't performing the way it used to.

You may find yourself searching for a word during a presentation, rereading an email several times, losing your train of thought during a meeting, or needing more time to organize your thoughts. These changes can affect productivity, confidence, relationships, and how you experience yourself at work.

The distinction is not whether brain fog affects your life. It is how the cognitive changes present, whether they are progressive, and whether they increasingly impair your ability to function independently.

Subjective cognitive complaints during the menopause transition are not automatically evidence of dementia.

Your brain is changing, too.

The menopause transition is a neuroendocrine transition, not simply a reproductive one. Sex hormones have actions throughout the brain, and changes in hormone signaling occur alongside changes in other systems that influence cognition.

And hormones are only part of the picture.

Sleep disruption, low mood, stress, fatigue, hot flashes, and reduced physical activity can all affect attention, concentration, memory, and executive functioning. These symptoms can be particularly prominent during the menopause transition.

Put those factors together with changing hormone levels, and it is understandable that you may not feel as mentally sharp as you once did.

What about dementia?

Dementia is different because the cognitive decline is progressive and can increasingly interfere with everyday functioning and independence.

Changes that warrant medical evaluation include progressively worsening memory or thinking, difficulty completing familiar tasks, problems managing finances or medications, getting lost in familiar places, significant changes in judgment, or changes in language, behavior, or personality.

And while menopause-related brain fog is common, it is important not to assume that every cognitive change is caused by menopause.
